These Terms of Service ("Terms") govern your use of CoverageCheck by Lonia AI ("CoverageCheck," "the Service"), a Medicaid eligibility denial review service operated by Lonia AI ("we," "us," or "our"). By creating an account or using the Service, you agree to these Terms.
If you do not agree to these Terms, do not use the Service.
1. Description of Service
CoverageCheck is a document analysis service. You upload your Medicaid denial or disenrollment notice and supporting documents. We review them against federal and state Medicaid eligibility rules and required procedural protections, and we deliver a written report explaining what happened, whether errors were made, and how to appeal. When errors are found, the report includes a pre-drafted appeal letter you may use.
2. Not Legal Advice
CoverageCheck is not a law firm. CoverageCheck does not provide legal advice or legal representation.
Our reports are document analysis and eligibility review prepared for informational purposes. Filing an appeal is your action, not ours. For complex cases, or any case where you want representation, consult a licensed attorney or your local legal aid organization. Use of CoverageCheck does not create an attorney-client relationship.
3. Eligibility
You must be at least 18 years old to use CoverageCheck on your own behalf. If you are submitting documents on behalf of a minor or another adult, you represent that you have legal authority to do so.
4. Account Registration
CoverageCheck uses OAuth single sign-on (Google or Microsoft) for authentication. You are responsible for maintaining the security of your OAuth account and for all activity that occurs under your CoverageCheck account.
5. Fees and Payment
A single review costs $49. Partner pricing is available for organizations that process multiple reviews per month. All payments are processed by Stripe and are charged in U.S. dollars.
6. Refunds
If CoverageCheck is unable to deliver your report within 5 business days due to a fault on our side, you may request a full refund. If your report is delivered, the fee is non-refundable. Disputes about a specific report should be raised by emailing admin@lonia.ai.
7. Your Documents
You retain all rights to documents you upload. You grant us a limited license to process those documents solely for the purpose of generating your report. Documents are deleted automatically after report delivery, as described in our Privacy Policy.
8. Acceptable Use
You agree not to:
- Upload documents that are not yours and that you do not have permission to share
- Submit fraudulent or fabricated documents
- Use the Service to harm any third party
- Attempt to gain unauthorized access to CoverageCheck systems
- Reverse-engineer, scrape, or republish our reports for resale
9. No Warranty
CoverageCheck is provided "as is" and "as available." We do not guarantee that any specific appeal will succeed. We do not guarantee that our analysis will identify every possible error in a denial. Medicaid policy is complex, varies by state, and changes over time. We make a good-faith effort to be accurate and current, but we cannot guarantee the outcome of any appeal you file.
10. Limitation of Liability
To the maximum extent permitted by law, Lonia AI's total liability arising out of or relating to your use of CoverageCheck is limited to the amount you paid for the specific report at issue. We are not liable for indirect, incidental, special, or consequential damages, including loss of coverage, denial of an appeal, or any other adverse outcome of your interaction with the Medicaid program.
11. Termination
You may stop using CoverageCheck and delete your account at any time. We may suspend or terminate accounts that violate these Terms. Termination of your account does not refund fees already paid for delivered reports.
12. Changes to These Terms
We may update these Terms as CoverageCheck evolves. Material changes will be communicated by email and reflected in the "Last Updated" date above. Continued use of CoverageCheck after the effective date constitutes acceptance of the revised Terms.
13. Governing Law
These Terms are governed by the laws of the State of New Jersey, without regard to its conflict of laws provisions. Any dispute arising out of or relating to these Terms or your use of CoverageCheck will be resolved in the state or federal courts located in New Jersey.
