CoverageCheck by Lonia AI ("CoverageCheck," "we," "us," or "our") is a Medicaid eligibility denial review service operated by Lonia AI (admin@lonia.ai). This Privacy Policy explains what data we collect, how we use it, and your rights regarding that data.

CoverageCheck is built around a simple principle: your medical and eligibility documents are not our data. We process them to produce your report, and we delete them once that report is delivered.

What We Collect

Account Information

When you sign in to CoverageCheck, we collect the information provided by your OAuth provider (Google or Microsoft):

• Your name
• Your email address
• Your profile photo, if available

We do not collect or store passwords. CoverageCheck uses OAuth single sign-on exclusively for authentication.

Payment Data

Payment processing is handled entirely by Stripe. We do not collect, store, or have access to your credit card number, bank account details, or other payment instruments. Stripe provides us with:

• Confirmation that your $49 payment cleared
• The transaction date and amount
• The last four digits of your payment method, for your reference in your account history

Documents You Upload

CoverageCheck deletes your uploaded documents automatically once your report is delivered.

When you submit a denial review request, you upload documents that may include your Medicaid denial or disenrollment notice, income documentation, and prior enrollment confirmation. These documents are:

• Encrypted in transit (TLS) and at rest
• Stored only in tenant-isolated, access-controlled storage
• Used solely to generate your report
• Automatically and permanently deleted after your report is delivered

We do not share your uploaded documents with third parties. We do not use them to train AI models. We do not retain copies after report delivery.

Your Report

Your report itself is retained in your account for the duration of your account so that you can download it again if you need to. You may request deletion of your report at any time. If you delete your account, your report is also deleted.

Audit Logs

We maintain audit logs of significant actions on your account: sign-in, document upload, payment, report generation, and report download. Audit logs do not contain document content. They exist for security and compliance purposes and are retained for a reasonable security window.

Usage Analytics

We may collect anonymized, non-personally-identifiable usage data to improve the service, such as:

• Pages visited
• Browser type and screen size
• Error reports, which do not contain document content

We do not use third-party advertising trackers. We do not sell, share, or monetize your usage data.

How We Use Your Information

We use the information we collect to:

• Authenticate your identity and manage your account
• Process your $49 payment through Stripe
• Generate your eligibility review report
• Deliver your report to you
• Communicate with you about your specific report (delivery, follow-up questions)
• Maintain security and detect abuse

HIPAA

CoverageCheck is not a covered entity or business associate under HIPAA, because we do not provide treatment, payment, or healthcare operations on behalf of a covered entity. We apply HIPAA-grade safeguards (encryption, access control, audit logging, minimum necessary access) as a matter of principle, not legal obligation.

Your Rights

You may, at any time:

• Access the information we hold about you
• Request correction of inaccurate information
• Request deletion of your account and associated data
• Request a copy of your data in a portable format
• Withdraw consent for any optional data processing

Children

CoverageCheck is not directed to children under 13. We do not knowingly collect personal information from children under 13. If you believe a child has provided us with personal information, please contact us so we can remove it.

Changes to This Policy

We may update this Privacy Policy as CoverageCheck evolves. Material changes will be communicated by email and reflected in the "Last Updated" date above. Continued use of CoverageCheck after the effective date of a revised policy constitutes acceptance of the changes.
